
Children’s mouths are constantly developing, and sometimes a small band of tissue can interfere with that process. Known as a frenum, this tissue connects the lips, cheeks, or tongue to surrounding structures. When a frenum is unusually tight, thick, or restrictive, it may limit normal movement. A frenectomy removes or releases the restrictive tissue and may help improve oral function.
But how do you know if your child might need one? Here are five signs worth discussing with their dentist.
1. Difficulty Breastfeeding or Eating
For babies, a restrictive tongue or lip tie can make breastfeeding challenging. You may notice that your baby has trouble latching, frequently loses the latch, makes clicking sounds while feeding, or seems unable to move their tongue effectively.
As children get older, oral restrictions can sometimes make chewing or moving food around the mouth more difficult. While feeding challenges can have many causes, persistent problems deserve an evaluation.
2. Speech Difficulties
The tongue plays an important role in producing many sounds. If its movement is significantly restricted, some children may have difficulty forming certain sounds or articulating words clearly.
A tongue tie does not automatically mean a child needs a frenectomy, though. Speech development varies considerably between children, and a speech-language pathologist may be part of the evaluation. If restricted tongue movement appears to be contributing to a speech concern, your child’s dental team can help determine whether treatment may be appropriate.
3. Trouble Moving the Tongue
One of the most noticeable signs of a restrictive tongue tie is limited tongue mobility. Your child may have difficulty sticking their tongue out, lifting it toward the roof of their mouth, or moving it from side to side.
You might also notice that the tip of the tongue appears heart-shaped when your child sticks it out. This can happen when a tight frenum pulls on the tongue.
4. A Gap Between the Front Teeth
A prominent upper lip frenum can sometimes contribute to a gap between the two upper front teeth, known as a diastema. Not every gap is caused by a restrictive frenum, and gaps are also a normal part of childhood dental development.
However, if a child has a pronounced frenum attachment and a persistent gap, their dentist may recommend monitoring the area or considering treatment depending on their age and overall dental development.
5. Oral Hygiene Challenges
Restricted movement can sometimes make it harder for children to clean certain areas of their mouths effectively. A prominent or tight frenum may also make brushing around the tissue uncomfortable.
Good oral hygiene is especially important while children are developing their permanent teeth. If your child consistently struggles to brush or keep certain areas clean, mention the concern during their next dental appointment.
Does Your Child Need a Frenectomy?
These signs do not necessarily mean your child needs a frenectomy. Some restrictive frenums cause no problems and may not require treatment at all. A thorough evaluation can help determine whether the tissue is actually affecting your child’s oral function and whether releasing it could provide a meaningful benefit.
